Privacy Policy
Effective date: October 26, 2026
Important notice
This Privacy Policy is provided as a draft for review. It does not constitute legal, regulatory, or HIPAA compliance advice. Because Eagle Eye Transformation Clinic collects health-related information, you should have this policy reviewed and approved by a qualified healthcare attorney or compliance professional before publishing it.
Information we collect
When you book a free discovery call or otherwise contact us, we may collect:
- Contact information: name, email address, phone number, and preferred contact method.
- Health and wellness information: any medical history, symptoms, weight-loss goals, current medications, allergies, or other health details you voluntarily share before or during the discovery call.
- Appointment details: scheduling preferences, call notes, and communications with our clinical team.
- Technical information: IP address, browser type, device information, and cookies used to keep the site secure and functional.
How we use your information
We use the information you provide to schedule and conduct your consultation, coordinate your care, respond to your questions, and communicate follow-up information about programs, supplements, or services you have expressed interest in. We do not sell your personal or health information to third parties.
How we store and protect your information
We store personal and health information using secure, access-controlled systems. Access is limited to authorized clinical and administrative staff who need the information to provide care or run the clinic. We use encryption-in-transit and industry-standard security practices to protect against unauthorized access, loss, or misuse.
No system is completely risk-free. If we become aware of a breach that affects your information, we will notify you in accordance with applicable law.
Discovery-call booking and Calendly
Our free discovery calls are booked through Calendly, a third-party scheduling service. When you schedule through Calendly, you may be asked to provide your name, email, phone number, and any optional notes or questions. Calendly processes this information on our behalf to confirm the appointment and send reminders.
HIPAA and Business Associate Agreements: Whether Calendly is HIPAA-compliant for our specific use depends on whether we have a signed Business Associate Agreement (BAA) with Calendly and are using an eligible Calendly plan and configuration. Calendly offers a BAA addendum for qualifying covered entities. We are actively evaluating our scheduling workflow to determine whether a BAA is required and in place. Please contact us if you need confirmation of our current BAA status.
For Calendly's own privacy practices, please see Calendly's Privacy Policy.
Third-party services and payment processors
In addition to Calendly, we may use trusted service providers for website hosting, analytics, email communication, payment processing, and e-commerce. These providers only receive the information needed to perform their specific function and are contractually required to protect it. We do not authorize them to use your information for their own marketing purposes.
Your rights and choices
You may request access to, correction of, or deletion of the personal information we hold about you. You may also opt out of marketing communications at any time. To make a request, contact us using the information below. We will respond within the timeframe required by applicable law.
Changes to this policy
We may update this Privacy Policy as our services, partners, or legal obligations change. The effective date at the top of this page will be revised accordingly. We encourage you to review this page periodically.
